Normal style='text-indent:21.0pt;line-height:150%'>1.1本招標項目蒲江縣人民醫(yī)院整體遷建項目和人民醫(yī)院康復中心及配套設施建設項目（二標段）施工 / 標段已由蒲江縣發(fā)展和改革局以蒲發(fā)改發(fā)【2019】第 12 號、蒲發(fā)改發(fā)[2018]26 號批準建設，項目業(yè)主為蒲江縣人民醫(yī)院，建設資金來自國家投資-政府投資，項目出資比例為100%，招標人為成都市青蒲小城鎮(zhèn)建設投資有限公司。項目已具備招標條件，現(xiàn)進行公開招標。

Normal align=left style='margin-right:-1.0pt;text-align:left; text-indent:15.75pt;line-height:150%;text-autospace:none'>2.2建設規(guī)模：總建筑面積約94491平方米,本項目包含1棟門診醫(yī)技住院綜合樓、1棟后勤樓、1棟學術交流中心、總平景觀等內(nèi)容。其中門診醫(yī)技住院綜合樓,地上12層,地下1層,總建筑高度53.7米；后勤樓地上3層，地下1層，總建筑高度12.5米；學術交流中心地上1層，總建筑高度7.3米。

Normal style='text-indent:21.0pt;line-height:150%'>3.1.1須具有 國家建設行政主管部門頒發(fā)的建筑工程施工總承包二級及以上資質(zhì)；
Normal style='text-indent:21.0pt;line-height:150%'>3.1.2信譽要求：不存在投標人須知第1.4.3項規(guī)定的限制投標的情形；
Normal style='text-indent:21.0pt;line-height:150%'>3.1.3企業(yè)注冊地不在四川省行政區(qū)域內(nèi)的省外企業(yè)須提供在有效期內(nèi)的四川省住房和城鄉(xiāng)建設廳官網(wǎng)已公開的入川信息網(wǎng)頁截圖（如投標人為聯(lián)合體，則聯(lián)合體所有成員均應具有）。
Normal align=left style='text-align:left;text-indent:21.0pt; line-height:150%'>3.1.4所有投標人（無資質(zhì)企業(yè)除外）應具有針對本標段的《成都市建設領域市場主體信用記錄（評價）網(wǎng)絡登記表》、《成都市建設領域從業(yè)人員信用記錄（評價）網(wǎng)絡登記表》。
Normal style='text-indent:21.0pt;line-height:150%'>3.1.5 擬派項目經(jīng)理（項目負責人）具有建筑工程專業(yè)一級或以上注冊建造師證書，須具備有效的安全生產(chǎn)考核合格B證，/（業(yè)績要求），須為本單位人員（若為聯(lián)合體投標，須為聯(lián)合體牽頭人人員）。

Normal style='text-indent:21.0pt;line-height:150%'>近年（2021年01月01日至投標截止時間，不少于3年）已完成或正在施工或新承接的項目共不少于1個類似工程業(yè)績。
Normal style='text-indent:21.0pt;line-height:150%'>類似工程業(yè)績是指:單項合同金額不低于12000萬元的房屋建筑工程施工業(yè)績
Normal style='text-indent:21.0pt;line-height:150%'>3.2本次招標不接受聯(lián)合體投標。
Normal style='text-indent:21.0pt;line-height:150%'>3.3 各投標人均可就上述1個標段投標，可中標的標段數(shù)量為1個。
Normal style='text-indent:21.0pt;line-height:150%'>3.4入圍條件：房建施工投標人開標當日在“成都市建筑市場信用信息管理系統(tǒng)”中被列入市場禁入名單的（聯(lián)合體投標的，組成聯(lián)合體各投標人任意一家被列入市場禁入名單的），在交易云平臺勾選主體信用評價的專業(yè)類別與其投標使用的資質(zhì)類別不一致的，一律不能成為合格投標人。

Normal style='text-indent:21.0pt;line-height:150%'>4.1凡有意參加投標者，請于2024年09月22日起，在成都市公共資源電子交易云平臺(www.cdggzy.com）（以下簡稱“電子交易云平臺”）免費下載招標資料（招標文件及相關資料），獲得“擬投標項目回執(zhí)”。

Normal style='line-height:150%'>5．投標文件的遞交
Normal style='text-indent:21.0pt;line-height:150%'>5.1 投標文件遞交的截止時間（投標截止時間，下同）為2024年10月14日09時30分，招標人或其委托代理機構，以及已在投標截止時間前按規(guī)定上傳投標文件的投標人，均應在開標當日投標截止時間前登錄不見面開標系統(tǒng)（網(wǎng)址：www.cdggzy.com），錄入相關人員姓名、電話等基本信息。投標人未按時登錄不見面開標系統(tǒng)，錯過開標解密時間的，由投標人自行承擔不利后果。
Normal style='text-indent:21.0pt;line-height:150%'>5.2 投標人應在解密開始后30分鐘內(nèi)在線完成投標文件解密。投標文件未能在規(guī)定時間內(nèi)成功解密的，視為投標人未在規(guī)定時間內(nèi)提交投標文件，由投標人自行承擔責任。不見面開標項目不接受投標人現(xiàn)場遞交光盤等形式的投標文件，不接受投標人現(xiàn)場解密投標文件。
